Card on file
Payment is due the day of the visit. A card on file also covers late cancellations per practice policy.
Online card vault is not enabled on this host yet (Stripe keys not set). Until then:
- Download the credit card authorization PDF.
- Complete and return securely as instructed after booking.
- Or call the practice at 773-312-3858.
